Austin Center for Therapy and Assessment, LLC

An unencrypted laptop, containing the electronic protected health information (ePHI) of 1,870 individuals, was stolen from the covered entity's (CE) office. The ePHI involved includes clinical evaluation reports, test results, patient names, addresses, phone numbers, and social security numbers. Upon discovery of the breach, the CE notified affected individuals, OCR and the media. Following OCR's investigation, the CE revised its HIPAA policies and procedures, implemented additional physical safeguards in its facility and installed encryption software.
