OCR description
A newly hired employee impermissibly took patient registration documents home. The records taken included the protected health information of 551 patients. The information at issue included names, addresses, birth dates, social security numbers, and driver's license numbers. As a result, the CE terminated the employee, provided notice to the affected individuals, amended registration procedures, implemented additional safeguards for such information, and offered identity theft protection to the affected individuals.
