OCR description
Titus Regional Medical Center, the covered entity (CE), reported the theft of the protected health information (PHI) of an undetermined number of individuals from an offsite storage location. The PHI involved in the breach included first and last names, medical record numbers, account numbers, and in some cases, doctor’s reports. The CE filed a police report and provided breach notification to HHS, affected individuals, and the media. The CE also provided additional training to the involved employees. As a result of OCR’s investigation, the CE conducted a risk assessment and implemented additional safeguards for records contained in the storage location.
