OCR description
Two laptop computers containing the protected health information (PHI) of approximately 540 individuals were stolen during a break-in at the offices of the covered entity (CE), Original Medicine Acupuncture & Wellness. The CE provided breach notification to HHS, affected individuals, and the media. Following the incident, the CE upgraded its security system and reduced the number of computers that maintains PHI. OCR reviewed copies of the CE’s relevant HIPAA policies and procedures.
