OCR description
On August 1, 2013, the covered entity (CE), Dr. Benkelman, discovered that her unencrypted office laptop computer had been stolen from her unlocked office. The resulting breach affected approximately 1,500 patients, and the electronic protected health information (ePHI) included demographic and mental health information (diagnoses/conditions). The CE reported the theft to the police, and provided breach notification to HHS, the media, and affected individuals. The CE also offered credit monitoring to affected individuals. The CE closed the practice in the fall of 2013 due to the breach.
