OCR description
The covered entity (CE), Molina Healthcare of Texas, Inc., inadvertently mailed Children Health Insurance Plan (CHIP) identification (ID) cards to the wrong households, affecting 2,826 individuals. This occurred due to a mismatch between program ID numbers in the CE’s system after the ID numbers were changed. The types of protected health information involved in the breach included names, addresses and other identifiers. The CE provided breach notification to HHS, affected individuals, and the media. Following the incident, the CE re-programmed its software and re-issued ID Cards to the affected individuals. Additionally, the CE offered 12 months of free identity theft protection services. OCR obtained assurances that the CE implemented the corrective actions listed.
