OCR description
On May 30, 2014, an unencrypted laptop computer was stolen from a company closet. The laptop contained the protected health information (PHI) of approximately 982 individuals, including names and data from medical tests. The covered entity (CE) provided breach notification to HHS, affected individuals, and the media and also notified police. Following the breach, the CE sanctioned and retrained the employee responsible for securing the computer and implemented new policies and procedures to improve safeguards to PHI. OCR obtained written assurances that the CE implemented the corrective actions listed above.
