OCR description
Printed pharmacy reports containing protected health information (PHI) about patients’ prescriptions was disclosed to an acquaintance of a former pharmacy employee in Sebring, Florida. The PHI involved in the breach included the names, addresses, prescribers, and medications for approximately 1,866 individuals. The CE provided breach notification to HHS, affected individuals, and the media. The CE also contacted law enforcement and reinforced with the pharmacy staff the CE’s HIPAA policies and procedures pertaining to the appropriate use, disclosure, and the safeguarding of PHI. OCR obtained written assurances that the CE implemented the corrective actions listed above.
