OCR description
The covered entity (CE), Owensboro Medical Practice, PLLC, reported that three employees of its business associate (BA) took a spreadsheet containing the protected health information (PHI) of 4,077 individuals. The PHI involved included names, addresses, phone numbers, dates of birth, Social Security numbers, and treatment information. The CE notified HHS, affected individuals, and the media. Following the breach, the CE and BA implemented additional administrative, technical, and physical safeguards to better protect sensitive data. All employees were retrained. OCR obtained assurances that the CE and BA implemented the corrective actions noted.
