OCR description
NYU School of Medicine, the covered entity (CE), reported that an employee inadvertently mailed a patient an unencrypted CD containing the electronic protected health information (ePHI) of 834 individuals. The ePHI involved included names and treatment information. The CE notified HHS, affected individuals, and the media. In its mitigation efforts, the CE implemented additional administrative safeguards and retrained its staff.
