OCR description
Vandals broke into a building storing paper protected health information (PHI) for the covered entity (CE), Hunt Regional Medical Partners. The types of PHI involved in the breach included patients' names, addresses, dates of birth, social security numbers, claims information, and patients' chart information. Approximately 3,000 individuals were affected. Upon discovering the breach, the CE filed a police report. The CE provided breach notification to HHS, affected individuals, and the media. The CE improved physical safeguards and retrained staff. OCR obtained assurances that the CE implemented the corrective actions listed.
