OCR description
The covered entity (CE), Clinical Reference Laboratory, Inc., sent a parcel which was damaged and opened during the mailing process by the United States Postal Services (USPS). The protected health information (PHI) involved in the breach included the names, dates of service, partial social security numbers, and lab test types of approximately 4,668 individuals. The CE provided breach notification to HHS, affected individuals, and the media. Since multiple breach reports have been received involving the same CE and fact pattern, this investigation was consolidated into one investigation.
