Jacobi Medical Center

The covered entity (CE), New York City Health and Hospitals Corporation - Jacobi Medical Center, reported that a former employee sent emails to her personal and professional email accounts that included the electronic protected health information (ePHI) of 90,060 individuals. The ePHI involved included names, addresses, dates of birth, telephone numbers, Social Security numbers, health insurance information, and treatment information. The CE notified HHS, affected individuals, and the media. In response to the breach, the CE implemented additional administrative and technical safeguards to better protect its ePHI.
