OCR description
The covered entity (CE), Woodhull Medical and Mental Health Center, reported that a laptop was stolen that contained the electronic protected health information (ePHI) of 1,581 individuals. The ePHI involved included names, dates of birth, gender, clinical information, and other treatment information. The CE notified HHS, affected individuals, and the media. In response to the breach, the CE implemented additional physical and technical safeguards. The CE also provided security awareness training to all workforce members.
