OCR description
A workforce member of Children’s Medical Clinics of East Texas, the covered entity (CE), took pictures of protected health information (PHI) displayed on a workstation computer and disclosed the pictures to a former workforce member. The PHI potentially included names, dates of birth, and the diagnoses and treatment information of 15,916 individuals. Upon discovering the breach, the CE filed a police report. The CE provided breach notification to HHS, affected individuals, and the media. The CE also improved physical security, administrative and technical safeguards and retrained staff. OCR obtained assurances that the CE implemented the corrective actions listed.
