Physicians Health Plan of Northern Indiana, Inc.

The covered entity (CE) mistakenly mailed protected health information (PHI) to unauthorized individuals following a folder/inserter machine error. Approximately 1,708 individuals that include all dependents of the CE's subscribers were affected by this breach. The erroneous billing statement mailing included names, addresses, PHP member identification numbers, and premium amounts. The CE provided breach notification to HHS, affected individuals, and the media. To prevent a similar breach from happening in the future, the CE implemented a formal audit checklist that requires independent verification by mailroom personnel. OCR obtained assurances that the CE implemented the corrective actions listed above.
