OCR description
An external hard drive containing the clinical and demographic information of approximately 2,300 individuals inadvertently fell into a garage can around November 5, 2015. The covered entity (CE), Grx Holdings, LLC dba Medicap Pharmacy, provided breach notification to HHS, affected individuals, and the media. In response to the breach, the CE adhered the external hard drives to the wall and initiated a change to eliminate the use of external hard drives as a data backup. It also sanctioned and retrained the involved employees. OCR obtained documentation that the CE implemented these corrective action steps.
