OCR description
On January 13, 2016, the covered entity (CE), Walgreens Pharmacy, reported that a theft took place at one of its stores located at 1350 Broadway in New York. The breach involved prescription numbers, first and last names, dates of birth, addresses, medication and insurance information for approximately 880 individuals. The CE provided breach notification to HHS, affected individuals and the media. Following the breach, the CE re-trained its pharmacy staff and sanctioned the employee whose action led to the breach. OCR obtained documented assurances that the CE implemented the corrective actions listed.
