OCR description
In January, 2015, a business associate (BA), Bizmatics, discovered that one of its computer servers was compromised by an unknown individual or individuals (hackers). The breach affected approximately 650 of the covered entity's (CE) patients. The CE cooperated with OCR and accepted the technical assistance provided until it closed for business in February 2017. Based on the foregoing, OCR decided not to further investigate.
