OCR description
An unauthorized user obtained remote access to the computer network located at the covered entity (CE), Medical Colleagues of Texas. The types of protected health information (PHI) involved in the breach included the names, addresses, social security numbers, driver’s license numbers, health insurance information, and medical treatment information of approximately 68,631 individuals. As a result of the breach, the CE improved safeguards, and updated policies and procedures. Further, the CE provided breach notification to HHS, affected individuals, and the media and provided free credit monitoring to affected individuals. OCR obtained assurances that the CE implemented the corrective actions listed above.
