OCR description
On April 4, 2016, the covered entity (CE), Coordinated Health Mutual, Inc. d/b/a InHealth Mutual, and its business associate (BA), HealthSCOPE Benefits, received communications from policy holders advising that they had received an incorrect IRS Form 1095-B in the mail. After researching the issue, it was determined that the issue resulted from faulty programming logic during the data compilation phase of the Form 1095-B development process. By order of the Ohio Department of Insurance on May 24, 2016, the CE was dissolved. Consequently, there is no longer a CE existing to be the subject of further investigation.
