OCR description
King of Prussia Dental Associates’ network server was hacked. The breach affected the electronic protected health information (ePHI) of 16,768 individuals and included names, dates of birth, social security numbers, and addresses, as well as clinical information. The covered entity (CE) provided breach notification to HHS, affected individuals, and the media. Following the breach, the CE strengthened its technical safeguards, including its firewalls and anti-virus protection. OCR reviewed the CE's risk analysis to ensure compliance with the Security Rule. The CE provided OCR with assurances it would continue to strengthen its technical safeguards.
