OCR description
The covered entity (CE), Singh and Arora Oncology Hematology, reported that a server used by its former business associate experienced a cyber-attack that compromised the protected health information (PHI) of 22,000 individuals. The PHI involved included names, addresses, dates of birth, Social Security numbers, and claims information. The CE notified HHS, the affected individuals, and the media. In its mitigation efforts, the CE offered free credit monitoring services and implemented additional administrative, technical, and security safeguards to better protect its PHI.
