OCR description
The business associate (BA), Ambucor Health Solutions, filed a separate breach report for an incident also reported by this covered entity, (CE), Akron General Medical Center. OCR obtained a copy of the BA agreement between this CE and BA and a copy of the breach notification letter sent to the affected individuals. This case has been consolidated into the other review of the BA.
