Seguin Dermatology, Office of Robert J. Magnon, MD

An unauthorized user obtained remote access to the computer system of the covered entity (CE), Seguin Dermatology. The protected health information (PHI) potentially affected included the names, addresses, dates of birth, and social security numbers of approximately 29,969 individuals. As a result of the breach, the CE improved safeguards, updated its policies and procedures, and trained its workforce members on better practices to protect PHI. The CE provided breach notification to HHS, affected individuals, and the media. OCR obtained assurances that the CE implemented the corrective actions listed.
