OCR description
The covered entity (CE), State of New Hampshire, Department of Health and Human Services, reported that a former patient accessed the electronic protected health information (ePHI) of 14,947 individuals. The ePHI involved included names, dates of birth, addresses, e-mail addresses, Social Security numbers, and health insurance information. The CE notified HHS, affected individuals, and the media. In response to the breach, the CE implemented and updated its policies and procedures to better protect its ePHI..
