OCR description
On April 4, 2017, the covered entity (CE), Carson Valley Medical Center in Gardnerville, Nevada, reported that a spreadsheet containing protected health information (PHI) may have been compromised by an unauthorized user as a result of an email scam. The electronic PHI included the names, discharge dates, billing account numbers, and locations of services for 11,368 individuals. The CE provided breach notification to HHS, affected individuals, and the media. Following the breach, the CE implemented technical safeguards, updated its security risk analysis, and trained staff. OCR provided technical assistance regarding the HIPAA Security Rule.
