Michigan Facial Aesthetic Surgeons d/b/a University Physician Group

The covered entity (CE), Michigan Facial Aesthetic Surgeons dba University Physician Group, reported that an employee’s laptop, containing the electronic protected health information (ePHI) of 3,467 individuals, was stolen. The ePHI involved included names, email addresses, mailing addresses, phone numbers, dates of birth, Social Security numbers, and treatment information. The CE notified HHS, affected individuals, the media, local law enforcement, and provided substitute notice. The CE established a complimentary phone number for questions or concerns. In its mitigation efforts, the CE revised its policies and procedures and retrained its employees. OCR obtained assurances that the CE implemented the corrective actions noted.
