OCR description
The covered entity (CE), New York City Health and Hospitals Corporation - Coney Island Hospital, reported that a volunteer accessed the protected health information (PHI) of 3,494 individuals. The PHI involved included names, dates of birth, and medical record numbers. The CE notified HHS, affected individuals, and the media. Following the discovery of the incident, the CE implemented additional administrative safeguards and retrained its staff.
