OCR description
A former dentist and dental hygienist of the covered entity (CE), Kennebunk Center for Dentistry, sent mailings to 1,900 patients, advertising the practice that they were joining. The patients’ names and addresses were taken from a call list that the dentist maintained off-site for emergency and night calls. The CE provided breach notification to HHS, affected individuals, and the media. As a result of the breach, the CE reviewed and retrained its staff on its policies and procedures related to uses and disclosures, safeguards and the minimum necessary standard. OCR obtained assurances that the CE implemented the corrective actions noted above.
