The University of Vermont Medical Center

The covered entity (CE), University of Vermont Medical Center, reported that an employee was the victim of an email phishing attack. The breached account contained the protected health information (PHI) of 2,300 individuals. The PHI involved included names, addresses, dates of birth, clinical and diagnostic information, and Social Security numbers. As a result of this breach, the CE immediately reset the employee’s password and implemented additional technical safeguards to better protect its sensitive data. The CE notified HHS, affected individuals, and the media. Substitute notices was also provided. The CE provided complimentary credit monitoring services to affected individuals. Finally, the CE instructed its staff on the proper methods for identifying fraudulent email communications. OCR obtained assurances that the aforementioned corrective actions were implemented.
