OCR description
The covered entity, The Neurology Foundation, reported that an employee impermissibly transferred files from his work computer to his personal electronic devices. This breach affected the electronic protected health information (ePHI) of 12,861 individuals. The ePHI involved included names, addresses, phone numbers, email addresses, dates of birth, Social Security numbers, diagnoses, health insurance and financial information, medications prescribed, and other treatment information. The CE notified HHS, affected individuals, and the media. In response to the breach, the CE provided complimentary credit monitoring services and retrained its employees.
