OCR description
The covered entity (CE), the University of Wisconsin - Madison reported that employees mailed postcards containing the protected health information (PHI) of 1,000 individuals. The PHI involved included names, addresses, medications prescribed, and other clinical information. The CE provided breach notification to HHS, the media, and affected individuals. The CE also offered free identity protection services to all individuals affected by this breach incident, implemented additional administrative safeguards, and retrained its employees. The employees involved were sanctioned. OCR obtained assurances that the CE implemented the corrective actions noted.
