Medical Mutual of Ohio

The covered entity (CE), Medical Mutual of Ohio, reported that an employee inadvertently emailed the protected health information (PHI) of 6,119 individuals to the wrong email address. The PHI involved included names, addresses, dates of birth, Social Security numbers, health insurance information, clinical information, and treatment information. The CE notified HHS, affected individuals, the media, and provided complimentary credit monitoring and identity restoration services. Following the breach, the CE implemented additional administrative and technical safeguards to better protect its PHI. OCR obtained assurances that the CE implemented the corrective actions noted.
