OCR description
Arkansas Oral & Facial Surgery Center, the covered entity (CE), reported that ransomware encrypted its data, affecting the protected health information (PHI) of 128,000 individuals. The types of PHI involved in the breach included demographic and clinical information. As a result of the breach, the CE improved safeguards. The CE provided breach notification to HHS, affected individuals, and the media and provided affected individuals with complimentary identity protection coverage. OCR obtained assurances that the CE implemented the corrective actions listed.
