OCR description
The covered entity (CE), Arch City Dental, reported that an employee inadvertently disclosed the electronic protected health information (ePHI) of 1,716 individuals. The ePHI involved included names and email addresses. The CE notified HHS, affected individuals, the media, and posted substitute notice on its website. Following the breach, the CE implemented additional administrative safeguards and retrained its staff.
