OCR description
The covered entity (CE), Shop-Rite Supermarkets, Inc., reported that an employee improperly discarded an electronic signature device which contained the electronic protected health information (ePHI) of 12,172 individudals. The ePHI involved included names, telephone numbers, dates of birth, medications prescribed, and other treatment information. The CE notified HHS, affected individuals, and the media. In its mitigation efforts, the CE implemented additional administrative and technical safeguards to better protect its sensitive data. The CE also retrained its staff.
