The covered entity (CE), Indiana University Health, reported that an individual found a plastic bag containing billing paperwork which included protected health information (PHI), at an intersection in Muncie, Indiana. The CE retrieved the plastic bag from the individual and obtained an attestation that he did not use or disclose the PHI. This breach affected 1,399 individuals and the PHI involved included names, dates of birth, gender, diagnoses, procedures performed, and other treatment information. The CE notified HHS, affected individuals, the media, and posted substitute notice on its website. The CE also established a dedicated call center for questions or concerns. In its mitigation efforts, the CE sanctioned the responsible employee and implemented additional administrative and physical safeguards to better protect sensitive data, and retrained its staff. OCR obtained assurances that the CE implemented the corrective actions noted.
