Sports Medicine & Rehabilitation Therapy, Inc.

The covered entity (CE), Sports Medicine & Rehabilitation Therapy, Inc., reported that it was the victim of a ransomware attack that affected the electronic protected health information (ePHI) of 8,800 individuals. The ePHI involved included names, dates of birth, addresses, Social Security numbers, and other demographic information. The CE notified HHS, affected individuals, the media, and provided complimentary identity and credit monitoring services to all affected individuals. In response to the breach, the CE implemented additional administrative, technical, and security safeguards to prevent further breaches. OCR obtained assurances that the CE implemented the corrective actions noted.
