Oklahoma Department of Human Services

An unauthorized source accessed a state assessment computer server, located at and maintained by an outside vendor. The server contained state assessment information that included names and social security numbers of 47,000 current and former clients of the covered entity (CE), Oklahoma Department of Human Services. As a result of OCR’s technical assistance, the CE provided breach notification to HHS, affected individuals, and the media and posted substitute notification on its website. Due to OCR’s investigation, the CE entered into a business associate agreement with the outside vendor. OCR obtained assurances that the CE implemented the corrective actions noted above. The CE also began researching the possibility of creating a HIPAA "hybrid entity.”
