OCR description
Eye Physicians, the covered entity (CE), reported that it experienced a ransomware attack that compromised the protected health information (PHI) of 2,620 individuals. The PHI involved included names, birthdates, and treatment information. The CE notified HHS, affected individuals, the media, and provided substitute notice. In its mitigation efforts, the CE deployed additional administrative, technical, and security safeguards to better protect its sensitive data. Staff were retrained.
