OCR description
The covered entity (CE), Franciscan Physician Network of Illinois, reported that boxes containing the protected health information (PHI) of 23,000 individuals were missing. The PHI involved included names, birthdates, addresses, and health insurance information. In its mitigation efforts, the CE implemented additional administrative, security, and physical safeguards to better protect PHI.
