OCR description
Pharmacy Innovations, the covered entity (CE), reported that it was the victim of a cyber-attack involving the electronic protected health information (ePHI) of 1,205 individuals. The ePHI involved included names, addresses, birthdates, medications prescribed, financial information, and e-mail addresses. The CE notified HHS, affected individuals, and the media. In its mitigation efforts, the CE implemented additional administrative, technical, and security safeguards to better protect its sensitive data. OCR obtained assurances that the CE implemented the corrective actions noted.
