OCR description
NYU School of Medicine – Pediatric Surgery Associates, the covered entity (CE), reported that an employee improperly disposed of a binder containing the protected health information (PHI) of 2,158 patients. The PHI involved included names, birthdates, health insurance information, and clinical information. The CE notified HHS, affected individuals, the media, and provided complimentary identity protection services. In its mitigation efforts the CE implemented additional administrative safeguards and retrained its staff.
