OCR description
An unauthorized individual gained remote access to the covered entity’s (CE) computer network server between August 1, 2017, and December 7, 2017, affecting 871 individuals. The types of protected health information (PHI) involved in the breach included demographic, clinical, and financial information. The CE provided breach notification to HHS, affected individuals, and the media. OCR obtained documentation showing that the CE updated and strengthened its password requirements and implemented technical safeguards to reduce the risk of malware infection or hacking. OCR provided technical assistance on HIPAA Security Rule compliance.
