OCR description
Onco360 and CareMed Specialty Pharmacy, the covered entity (CE), reported that three employees fell victim to an email phishing scheme that affected the protected health information (PHI) of 53,173 individuals. The PHI involved included names, dates of birth, addresses, Social Security numbers, diagnoses, conditions, medications prescribed, and other treatment information. The CE notified HHS, affected individuals, and the media. In its mitigation efforts, the CE implemented additional technical safeguards to better protect its data.
