St. Peter's Ambulatory Surgery Center LLC - d/b/a St. Peter's Surgery & Endoscopy Center

The covered entity (CE), St. Peter's Ambulatory Surgery Center, LLC dba St. Peter's Surgery & Endoscopy Center, reported that it experienced a ransomware attack that affected the electronic protected health information (ePHI) of 134,512 individuals. The ePHI involved included names, dates of birth, addresses, Social Security numbers, diagnoses, and health insurance information. The CE notified HHS, affected individuals, the media, and posted substitute notice on its website. The CE also provided complimentary credit monitoring and identity protection services to affected individuals. In response to the breach, the CE formalized a security incident policy and procedure and implemented additional technical safeguards. OCR obtained assurances that the CE implemented the corrective actions noted.
