The covered entity (CE), ATI Holdings, reported that several employees were the victims of an email phishing scheme that affected the protected health information (PHI) of 45,800 individuals. The PHI involved included names, dates of birth, drivers’ license or state identification numbers, Social Security numbers, financial and health insurance information, diagnoses, medications prescribed, and other treatment information. The CE notified HHS, affected individuals, the media, and provided substitute notice. In its mitigation efforts, the CE implemented additional administrative, technical, and security safeguards to better protect its PHI. In addition, the CE retrained its workforce members on the proper methods of identifying fraudulent email communications.
