Primary Health Care, the covered entity (CE), reported that it was the victim of an email phishing attack. The affected accounts contained the protected health information (PHI) of 10,313 individuals. The PHI involved included names, addresses, dates of birth, drivers’ license information, Social Security numbers, claims information, financial information, diagnoses/conditions, and treatment information. The CE notified HHS, affected individuals, and the media. As a result of this breach, the CE immediately suspended the affected accounts and implemented additional technical safeguards and retrained its staff. OCR obtained assurances that the CE implemented the corrective actions noted.
