OCR description
The covered entity (CE), Henry Ford Health System, reported that an employee lost an unencrypted flash drive containing the protected health information (PHI) of 1,658 individuals. The PHI involved included names, dates of birth, lab results, and treatment information. The CE notified HHS, affected individuals, the media, and posted substitute notice on its website. The CE sanctioned the responsible employee and retrained this individual on the importance of patient privacy and the security of sensitive data. OCR obtained assurances that the CE implemented the corrective actions noted.
